We're open Monday - Friday, 9am to 5pm

1. Fill out your form with your information
2. Bring 1 copy of your Live Scan Form 8016
2. Bring your valid ID
3. Payment via cash or credit card
4. We will make 2 copies of your form for you
5. Give 1 copy of the form to the agency that requested the live scan

Process is 5 to 10 minutes
