To continue our goal of disseminating useful information - Patients, Care Givers, HR & Insurance Agents need to know what discharge planning looks like and steps to take:
1) identify what care is needed next,
2) pull your list of in-network facilities or providers,
3) start calling - this will be time-consuming!
4) inquire about type of care provided, what age groups do they serve, do they have the needed specialty and space in their program.
5) If no space, or no space with the needed specialty, is available - your State may have network adequacy and access to care requirements! Work with your agent on this!

This will take a lot of time and it's worth it - continuity of care and fewer gaps in treatment lead to better outcomes!
