In the most recent weekly message from ISBE, they have outlined the changes to Form 11-01, which was updated in February of 2026.

Updated Physical Restraint and Time Out Form
An updated version of the Physical Restraint and Time Out Form (11-01) has been posted on ISBE’s Physical Restraint, Time Out, and Isolated Time Out webpage under “Resources.”
Updates have been made to questions 11, 12, and 17 on the form.
Question 11 – The language has been revised from “a certified staff person knowledgeable about the use of time out or trained in the use of physical restraint must evaluate the situation” to “an adult trained in compliance with 23 Ill. Admin Code 1.285 (h) must assess whether the student has ceased presenting the specific behavior for which the time out, isolated time out, and/or physical restraint was imposed.” Additional changes include a reminder that “In no event shall supine physical restraint last longer than 30 minutes unless continuation is authorized by a school administrator.”
Question 12 – An N/A checkbox was added to the Prescribed Medication line. The individual filling out the 11-01 Form should select the “Yes” box if the student is prescribed medication and has access to prescribed medication during the physical restraint, time out, and/or isolated time out. If the student is prescribed medication but did not have access to the prescribed medication during the physical restraint, time out or isolated time out, the individual should select “No.” Finally, if the student is not prescribed medication, then the individual completing the form should select “N/A.”
Question 17 – Space was added under Same Day Parent/Guardian Notification for staff to input additional information. ISBE has also included three boxes to remind staff to send a copy of the RTO Bill of Rights, Procedural Safeguards, and Information on Filing a State Complaint to the parent/guardian.
