EverCare Choice (formerly Elant Choice) understands how important staying in your home is to you. Our team of managers and healthcare professionals has remained in place, and maintains its commitment to providing the same exceptional Managed Long-Term Care services that our members in Orange, Rockland, and Dutchess counties have come to know and trust. We provide an array of covered long- and short-term care services through our existing network of trusted providers. Covered services include (but are not limited to):

Whether managing an existing condition, recovering from an injury or illness, or working to restore self-sufficiency, our professionals consult closely with you and your physicians to create an individualized care plan that meets your needs and addresses your wishes. Our goal is to help you get back to the lifestyle you enjoy, as quickly and safely as possible. EverCare's CHHA services include:

EVERCARE FOR THE DAY: ADULT SOCIAL DAY PROGRAM
EverCare for the Day wants you to enjoy peace of mind, knowing your loved one is safe, happy, and enriched. Our Adult Social Day Program, hosted in our handicapped-accessible, single-level space, provides endless opportunities to stay sharp and fit, and to feed your soul with a wide range of activities. Led by our team of professionals in a secure, comfortable environment, everything we do is designed to foster optimal well-being, and to decrease, delay, or prevent functional decline. Daily activities may include:
