We’ve achieved outcomes that truly reflect the power of person-centered care and built connections with members, families, providers and legislators that will continue to shape the future of care for people with I/DD across New York State.

While this chapter of PHP is coming to a close, our mission is not. Our team at Care Design New York will continue to champion and advocate for the I/DD community. Our commitment to integrated, whole-person care — and to the people we serve — will continue to guide everything we do.
