Secondary School Athletic Trainers... We need your help!
We invite you to participate in a research questionnaire intended to help us understand Athletic Trainer Mental Health Screening Practices in Secondary School (i.e., 9th, 10th, 11th, 12th grades) Pre-participation Examinations. You are being asked to participate because your input, knowledge, and responses are vital to understanding this phenomenon and informing the future of health care.

The questionnaire will take approximately 8-minutes to complete, and your participation is voluntary. All responses to this survey will remain confidential; no one will be able to identify you or your answers. There are no right/wrong answers to any questions. We ask only that you answer with what first comes to mind. If you do not feel a question is relevant to your experience, you may skip it. We hope you will answer most of the items, but you do not have to answer any question you do not want to answer for any reason at all.
The project has been certified exempt by the California State University, Fresno Institutional Review Board. By responding to the items, you are declaring that you are at least 18 years of age and grant permission to investigators to use your de-identified answers in this research.
