I am always careful about who gets to jump with me and that they are briefed on possible scenarios. Just because it doesn't happen often doesn't mean it cant happen to you. Drogue entanglement with a camera person. Do you know what the correct procedure is for both jumpers?
I find it frustrating that people don't jump with a hook knife on them. If you lose the one on your rig - replace it.
This small piece of equipment could end up saving your life. No one ever thinks these scenarios can happen to them - and yet it eventually may happen.

Even UPT has issued a tandem advisory that Instructors should have access to a knife after a recent fatality investigation.

So take safety seriously.
