If you saw me at the Javier Rd location at Eye Physicians and Surgeons owned by John Essepian, III MD, on or after July 1, 2020 your chart belongs to Eye Physicians and Surgeons near the beltway, and you can access your chart by calling them at 703-698-8880. I am no longer practicing at Eye Physicians and Surgeons.
