I was reading last night about NASA, pre-Challenger. It prided itself on open , a "tell it like it is" . Unfortunately, the postmortem on the disaster showed that candor only traveled laterally with peers and, of course, from down to their reports. Employees felt they couldn't tell their bosses problems, especially ones that might cancel a launch.
