Dearest Patients, Colleagues and Friends,

It is with heavy heart and profound sorrow that we share the news that Dr. David Graeme Shaw passed away unexpectedly of natural causes on 6-14-23.

We understand this will be a shock and will leave you with much sadness and questions for the future. We know the doctor would want you to know it is important to continue with appropriate medical care; therefore, we suggest establishing contact with your primary care physician or physician of your choice as soon as possible. Any current prescriptions with refills remaining may still be obtained at your pharmacy.

Finding a new physician may be difficult, thus we have listed below trusted physician referrals. Please feel free to contact them to assess the fit for your specific continuity of care.

For medical records please refer to your portal. Note: records from 2017-2020 are in storage and will need to be procured and downloaded; then faxed or emailed. Please allow time for this to occur. Additionally, please find on the website under the forms tab, the Medical Records Request Form in order to provide those records. A fee for records may apply.

Dr. Shaw had his bay area practice for over 28 years and has been treating grateful patients for even far longer. He was loved and respected by all. Dr. Shaw’s family and Practice Manager are deeply saddened by his loss and wish to convey their appreciation for the thoughts we have received. We will do our best to assist you in this difficult transition.
