Licensed Vocational Nurse (LVN) – Care Coordination
(Not Remote–Working with Virtual CareTeam in Longview, TX)

Join our Team! PhyNet is seeking an LVN to support the care team in a office setting focusing on remote coordination of care for patients with chronic conditions. This role emphasizes follow-up, care plan support, and maintaining continuity across care settings.

CORE DUTIES INCLUDE:

Conduct outreach to patients for chronic care follow-up, medication reviews, and health assessments.

Coordinate referrals, diagnostics, and provider communication across multiple care sites.

Update and maintain accurate records in the EHR.

Experience in chronic care management or care coordination preferred.
