Today, we want to spotlight our Reentry Hub! In 2025, our Hub program received 259 referrals, served 245 active clients, and provided compassionate, holistic support through 5,161 client interactions. Through the hard work of our reentry community health workers, 99% of clients' needs were successfully resolved.

We’re proud to highlight the incredible impact of Beacon’s health navigation program this past year. In 2025, we received 1,282 referrals, served 1,402 clients, and supported our community through 9,038 interactions. Thanks to the dedication of our community health workers, 97% of clients’ health-related social needs were successfully resolved.
