** There are no eligibility requirementswiththeexceptionthat:

1) Mothers must be pregnant and within 4 weeks of the expected delivery date; or
2) The infant is less than 1 year of age;
3) The parent/caregiver currently does not have a crib available for the infant;
4) The caretaker currently has no available resources to procure one on their own;
5) The caretaker must attend a Safe Sleep educational training session;
6) There are NO financial prerequisites.
