Did you know? Last week Moral Injury was Recognised in DSM-5-TR.
The DSM-5-TR has now formally recognized moral injury under the Z codes - those categories clinicians use to capture social and contextual issues that profoundly shape a person’s mental health.
📍 Importantly, this is not a new diagnosis.
Moral injury is listed as a “Moral, Religious, or Spiritual Problem” (Z65.8), alongside moral dilemmas and moral distress.
Why does this matter? Because the move places moral trauma on the clinical map. It signals that the distress of living with moral conflict whether from war, healthcare decisions, institutional betrayal, or other contexts is not just “in the background”. It is a legitimate focus of care.
For years, clinicians, veterans, chaplains, and researchers have been calling for this recognition. The APA has now taken the first step.
The implications are profound:
• Clinicians now have language to describe these wounds without pathologising them.
• Systems may be more accountable in addressing moral harms.
• Survivors can know their struggles are recognised as real and worthy of attention.
