CLASSIFICATION: Team Director
EFFECTIVE: Immediately
REPORTS TO: Director of Programs
GENERAL DESCRIPTION: Supervises, trains, and directs assigned staff, for the overall quality of care and service delivery to persons served within the team. Keeps administration appraised of any and all gaps in the agency's service delivery system. Provides recommendations for quality improvement and utilization review process and functions. Assures that work is performed in a manner that promotes person centered planning, self determination, professional standards and Wayne Center's code of ethics.
MINIMUM QUALIFICATIONS AND REQUIREMENTS:
One of the following:
A. A Master's Degree from an accredited college or university in Social Work, Sociology, Psychology, or Counseling - OR -
B. A Bachelor's Degree from an accredited college or university in Sociology, Psychology, Social Work, or Human Services, plus a Master's Degree in a related field, and a minimum of two years of experience as a clinician assessing/evaluating/developing clinical interventions for children.
Five years of experience in one of the fields named above, including a minimum of three years of experience as a human service professional.
Current certification and/or licensure as a LLMSW/LMSW, LLBSW/LBSW, LLPC/LPC or an LLP/LP to practice in the State of Michigan.
Three years of staff supervision preferred.
Knowledge of mental health, social services, and public health service systems.
Qualified Intellectual Disability Professional (QIDP) with specialized training and experience in treating or working with a person who has intellectual disability preferred.
Every year at the time of their licensing and/or credential renewal, evidence five hours of QIDP, five hours of Qualified Mental Health Professional (QMHP) and 24 hours of Children’s Mental Health Professional (CMHP) training (this applies to Children’s Team Director).
Experience with the team approach.
Demonstration of strong management, verbal and written communication skills.

POSITION DESCRIPTION
POSITION TITLE: Access/Intake Coordinator
REPORTS TO: Director of Programs
GENERAL DESCRIPTION: The Access/Intake Coordinator shall provide intake assessments/screenings to all children and adults referred to Wayne Center for services. Consumers may be entering the Detroit Wayne Integrated Health Network (DWIHN) for the first time, transferring from another or transferring from another Supports Coordination provider. Services may be provided in consumer homes, licensed and/or unlicensed settings as well as at various locations throughout Wayne County as contracted by DWIHN. This includes adherence to assessment rules established by Detroit Wayne Integrated Health Network (DWIHN), as well as all professional and ethical standards of their profession and all accrediting bodies. The Access/Intake Coordinator shall also perform various Customer Service duties to ensure the needs of all consumers/ members/recipients are being met.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Conduct intake assessments on all referred children and adults.
Complete all DWIHN required documentation relative to all intakes performed in a timely manner (including but not limited to Consents, Demographic Forms, Satisfaction Surveys, EPSDT, biopsychosocial assessments, etc.).
Enters all required intake information into Mental Health-Wayne Information Network (MH-WIN) electronic records and Lexicon within 2 working days of all intakes.
Assesses the clinical needs of each person at time of intake and clearly and concisely relates the clinical needs to the Supervisor prior to case assignment, alerting them to any potential service problems (ie…eligibility questions, lack of Medicaid, etc…).
Completes Intake Referral and forwards the indicator form to the Director of Programs for assignment to a Supports Coordinator within 3 business days of intake meeting.
Maintains MH-WIN scheduling calendar with daily updates regarding intake appointments, schedule availability, intake outcomes and all other required information.
Maintains adequate supply of all necessary intake forms, brochures, etc… and provides each new consumer with all required information in a Welcome Packet.
Assumes responsibility for gathering all data and typing Sentinel Event (SE) and Critical Incident (CI) Reports within 2 working days of receipt of Incident Reports.
Distributes copies of Incident Reports to assigned Supports Coordinator within 24 hours of receipt, date stamping each Incident Report (IR) with the date received.
Distributes copies of SE and CI reports to Psychologist(s) in accordance with policy for Behavior Management Committee reporting.
Prepares and maintains complete and thorough records of all Incident Reports received, Sentinel Events and Critical Incident Reports and compiles statistics for monthly report to the Authority within 5 business days of each new month for the previous month.
Maintains Annual files for all SE/CI reports, all Intake data and all monthly reports; ensuring that all reports are ready and available for audit purposes at all times.
Maintains guardianship hearing database
Petitions Wayne County Probate Court for appointment of guardianship
Coordinates Informed Consent Board (ICB) hearings for all guardianship petitioners, holding ICB hearings no more than 30 days from receipt of the request for ICB hearing.
Assembles and distributes an ICB schedule monthly (minimum) to all petitioning Support Coordinators, Team Directors and the Director of Programs.
Completes ICB packets, forwarding them to Director of Programs or guardianship coordinator for co-signature, within 5 business days of each ICB meeting.
Uploads completed ICB into guardianship
Maintains Customer Service Policy Manual for audit purposes and attends all annual and/or mandated Customer Service Trainings.
Performs all other duties as assigned by supervisor(s).
MINIMUM QUALIFICATIONS AND REQUIREMENTS:
The Access/Intake Coordinator must have earned at least a Master’s degree in Social Work, Psychology, or Counseling or other related human service field from an accredited college or university.
Must possess licensure as, Limited Licensed Masters Social Worker (LLMSW) Licensed Master Social Worker (LMSW), Limited Licensed Professional Counselor(LLPC), Licensed Professional Counselor (LPC), or Limited License Psychologist (LLP) without censure or licensure conditions (ie…probationary status).
Have basic computer skills; proficiency with Microsoft Word and the internet required.
Priority will be given to individuals who are credentialed (or are eligible for credentialing) as a Qualified Intellectual Disability Professional (QIDP) and as a Children’s Mental Health Professional (CMHP).
Adjusted hours and travel may be required in executing the duties and responsibilities of this position, e.g., travel to homes, schools, vocational settings, hospitals, medical offices, and other community locations as needed.
Extensive scheduled and non-scheduled travel is an essential function of this position. Employees are expected to provide their own transportation. Maintenance of a valid driver’s license, an acceptable driving record, and vehicle insurance in compliance with State of Michigan requirements are all continuing conditions of employment. If the employee must provide alternative forms of transportation, they are responsible for assuring that the alternative form of transportation complies with these directives.
EXPERIENCE:
Experience working with children and adults with Intellectual and Developmental Disabilities performing assessments, testing, and evaluation for a minimum of 2 years post-degree is required.
Experience working within the MHWIN system or Electronic Medical Record for a minimum of 1 year is preferred.
Experience performing Intake Assessments for a minimum of 1 year is preferred.
TRAINING:
Persons in this position are responsible for maintaining their professional license and meeting all training criteria established by Detroit Wayne Integrated Health Network and providing proof of such training upon request.
The Foster Care Caseworker shall provide services to all children referred by the Michigan Department of Health and Human Services (MDHHS) to Wayne Center's foster care homes. Services shall be provided in consumer, relative caregiver and foster parent homes. The Foster Care Caseworker must adhere to MDHHS rules and regulations, professional standards and ethical standards in the delivery of services/supports to all persons served. Services are performed in ways that assist each child to achieve permanency.
MINIMUM QUALIFICATIONS AND REQUIREMENTS:
1. Bachelors degree in Social Work, Psychology, or Guidance and Counseling or other related human service field from an accredited college or university is preferred.
2. Basic computer skills; proficiency with Microsoft Word preferred.
3. Successful completion of Child Welfare Training Institute (CWTI) requirements including a competence-based performance evaluation.
4. A minimum of 40 hours of CWTI pre approved and qualifying in-service training on an annual basis.
5. If transferring from another children's services position that has successfully completed the CWTI PSI training in that program, shall attend and complete CWTI Foster Care Private Agency Program Specific transfer training within six months of assuming this position.
6. New caseworkers who possess the University-based Child Welfare certificate shall be required to complete program-specific training. The program-specific training curriculum for Child Welfare certificate holders shall be reviewed and approved by MDHHS Contract Monitors.
