Crawford County Wellness Center Survey

Today (8/17) is our August food distribution! If you need food assistance, please visit the United Presbyterian Church of Denison from 3 to 5:30pm. Recipients will need to fill out The Emergency Food Assistance Program (TEFAP) form in order to receive assistance.

IMPORTANT UPDATE for July Food Distribution

Effective July 1, 2021 Crawford County food recipients will be required to fill out State provided paperwork verifying their eligibility to receive food designated for Crawford County residents. This form is based on The Emergency Food Assistance Program's (TEFAP) income guidelines and is required to be filled out every 12 months.

In addition, if you are picking up food for another family that is not present at the distribution, you must provide a TEFAP Eligibility Form and a Proxy Consent form with their signature authorizing you to do so.

The TEFAP form is for CCHF files and the Proxy Consent form is to be presented at each distribution.

The forms will be made available at our July 20th distribution, or may be picked up at the United Presbyterian, United Methodist, or Our Savior Lutheran church offices between 9:00am and 1:00pm on week days.
