You play such an important role in the health and well-being of our patients, and we are truly grateful for everything you do each day.

We also want to recognize our wonderful receptionists and front office staff — the first faces our patients see and the voices that help keep our office running smoothly from start to finish. From scheduling appointments and helping patients navigate their visits to multitasking through busy days with grace and positivity, you are such an essential part of our team. Your kindness, organization, and ability to handle it all with a smile make a bigger difference than you know.

Sports Physical Checklist:
-A parent or legal guardian must accompany the student to the sports physical if the student is 17 or under
-All forms provided by the school or sports league must be completed
-If the student wears prescription glasses or contacts, the vision aids must be worn or brought to the sports physical

***Please note that a sports physical does not replace your child's annual physical with their primary care physician or pediatrician***
