Are you a compassionate RN who enjoys building relationships, educating patients and families, and making a meaningful impact from day one? Unity is looking for an Admissions Registered Nurse to help guide patients and families through the transition to hospice and palliative care services.
In this role, you'll conduct comprehensive assessments, coordinate care needs, provide education and support, and collaborate with our interdisciplinary team to ensure patients receive the right care at the right time.
Primarily serving the Green Bay and Sturgeon Bay territories, with occasional travel throughout our service area.
