1) Subject Line: Football Fan

2) In the submission email include name, age range, height, weight, city and zip, phone, email.

3) Attach a current, right-now photo of your face and full body in JPEG format. Make sure the photos are taken with decent lighting and reflect your current hairstyle and facial hair (if any) as some of the scenes take place in different decades, so if you wear different hair styles/facial hair, please send photos that will represent your look on set.

*NOTE: Rename the title of your photos with your Full Name. You should do this with all your submission photos.

Bonus: If you have any Bears' apparel - please take the photos in that outfit. If you happen to have any apparel from the 1960s, 1970s, or Super Bowl eras, that is a double bonus!!!

4) This is a Christmas-themed film. The production will have water and cooling tents available, but scenes will require you to wear "winter attire" while filming in July. Are you willing to work in exterior scenes?

5) Please confirm your mode of transportation, i.e. Uber, Bus, Own Vehicle, etc.

6) Do you have any Bears' apparel? If yes, please give a brief but detail list. If you happen to have any apparel from the 1960s, 1970s, or Super Bowl eras, be specific, i.e. 1967 Gale Sayers jersey or Ditka’s Sweater Vest

7) Do you have any Bears themed tailgating items, i.e. Grills, Tents, Tables, Games, etc? Do you decorate your vehicle for a tailgate with a Bear theme? Please give a brief detail list or list N/A.

8) Not everyone will have to come in for a fitting, but please let us know if you are available the fitting dates. We will repeat some people at Solider Field, but not everyone has to work both dates, but please let us know which dates you are available to film.

We MUST have a current look that REFLECTS your current NATURAL hair!!! We know that some folks wear alternative hairstyles, but please upload the NATURAL look that you WEAR. You need to show up on set looking like the photo you used to represent your current look.
