A huge shoutout to the incredible team at Shawano Health Services for choosing Confidence After a Fall as their CTP Teal Living focus!

Their motto says it all:

This program is all about empowering residents, rebuilding confidence, and helping individuals regain independence after a fall. Through teamwork, education, and compassionate care, this amazing group is making a difference every single day.

We're excited to kick off CTP Teal Living Programs as Evergreen Health Services officially launches its chosen focus: Hospital to Home Bridge! 🏡

Every CTP Teal Living Program is designed to create meaningful experiences, improve outcomes, and strengthen the quality of life for residents. We can't wait to see the impact this team will make through their commitment to helping residents successfully transition from the hospital back home.
