The mission of Transform4Health is to achieve sustainable and measurable improvements in behaviors and health outcomes around to***co use of the citizens of Alexander, Cabarrus, Catawba, Cleveland, Gaston, Iredell, Lincoln, Rowan, Stanly and Union counties through the collaborative efforts of citizens, local government, and community organizations to implement policy and environmental change.
