Family Bridge Program

As we reflect on 2024, we are filled with gratitude for an incredible year at Family Bridge. We had the privilege of caring for over 1,000 patients, as they transitioned from hospital to home, helping newborns and parents adjust to life together, and celebrating significant milestones alongside our community and local leaders, including Governor Ned Lamont.

To close out the year, our team took a moment to relax and reflect on our journey with a celebratory paint party.

Thank you to all who have supported us — we look forward to the opportunities and challenges that 2025 will bring.

This season, Family Bridge is thankful for community. Our CHWs recently participated in Southern Connecticut State University Department of Public Health’s annual community day. Bridgeport residents were able to learn about the services our program offers, and even refer friends and family members who are currently pregnant. We’re grateful for our community and happy to serve. What are you thankful for? 🍁
