This case involves a 61-year-old woman with a bicuspid aortic valve—a congenital abnormality affecting ~2% of the population. Instead of the normal three leaflets, her valve has two, leading to severe calcific stenosis and an associated dilation of the ascending aorta (aortopathy).

The surgical plan:

⭕️Aortic valve replacement (bioprosthetic)

⭕️Ascending aortic replacement with a Dacron graft

⭕️Patent foramen ovale (PFO) closure through bicaval cannulation

This combined procedure not only restores valve function and prevents aneurysmal rupture, but also closes a potential shunt that can cause embolic complications.

What do you see when the gallbladder is pulled up during a lap chole?

With the right retraction, key structures come into view:
-The cystic duct and cystic artery to be dissected
-The common bile duct and duodenum just below
-A groove called Rouviere’s sulcus, when connected to the base of liver segment 4, helps avoid bile duct injury.
