Required Documents:
First Time Applicant Requirements- Provide proof of residence (Utility Bill, Financial Statement, etc.) and one of the following combinations:
· One primary and two secondary documents
· Two primary documents

Primary Documents
o Original Birth Certificate
o Passport

Secondary Documents
o Social Security card
o School ID (Middle School)
o Medical Insurance Card
o Original adoption papers
o Baptismal certificate

Signature of Parent or Legal Guardian
Parent or Guardian must also present the required documents.

Issuance Options (For Applicants 15 and Younger):
· 2-year ID: $13.00
· 6-year ID: $19.00
