The Center for Vaccines and Immunology at the University of Georgia are studying COVID-19 and SARS-CoV-2 infections and vaccinations. COVID vaccines began rolling out in December, 2020 and we are interested in your experiences with these novel vaccines. We developed a short survey to better understand any adverse events you may have experienced following vaccination.

The survey is voluntary and anonymous and information is confidential.
