Here are the Requirements if you are under DepEd Public schools.
1. Bio Page of the Philippine Passport
2. Original valid J1 Visa
3. Deped Clearance (to be emailed by Central Office to CFO)
4. Certificate of Eligibility for Exchange Visa (DS-2019)
5. Notarized Undertaking from the US Sponsor
6. Notarized Affidavit of Undertaking
7. Signed and Approved Health Insurance
