PDCA Problem-Solving Implementation Guide
1. Record the Problem
Clearly define the issue by addressing the following:

What? β Define the problem. Example: "Machine downtime due to overheating."

Where? β Specify the location. Example: "Production Line 3."
β
When? β Mention the time or frequency. Example: "Every 3 hours during peak operation."
β
Who? β Identify the team/person involved. Example: "Maintenance team and machine operators."
2. Analyze the Problem (Fishbone/Ishikawa Diagram)
Identify potential root causes under six categories:
1οΈβ£ Man (People) β Human-related issues like skill gaps. Example: "Lack of training on temperature monitoring."
2οΈβ£ Machine (Equipment) β Equipment failures. Example: "Cooling fan failure due to wear."
3οΈβ£ Management (Policies & Supervision) β Leadership and procedures. Example: "No preventive maintenance schedule."
4οΈβ£ Method (Process & Procedures) β Inefficient processes. Example: "Ineffective lubrication process."
5οΈβ£ Material (Resources) β Poor quality materials. Example: "Low-quality lubricants."
6οΈβ£ Milieu (Environment) β External factors. Example: "High temperatures affecting machine performance."
3. Identify Root Causes (5 Whys Technique)
Use the 5 Whys technique to drill down to the root cause:

β Why did the fan fail? β "Not replaced on time."
β Why was it not replaced? β "No preventive maintenance plan."
β Why is there no plan? β "Management did not prioritize it."
β Why did management not prioritize? β "Lack of awareness about maintenance importance."
4. Take Action (Corrective & Preventive Measures)
Define the actions and responsibilities:
