Passport Requirements for New applicants:
 Application form
 Philippines Statistic Authority (Birth and marriage certififcate)
 Local Certificate Register (Birth and marriage certificate)
 At least 2 government Id’s (SSS, Voters, PRC, driver’s license etc.)
If late Register additional documents:
 Form 137
 Year or annual book
 Bank Statement
 Utility bills (Meralco or Water bill)
 Baptismal
 Minimum of 5 growing up pictures

If Child (additional)

 Parents’ marriage contract
 Certificate of Enrollment
 Parent’s passport copies and id’s
 If the parent is not the guardian of the applicant, Special Power of Attorney is required.

 Old Passport
 2 government id (SSS, Voters, BIR, PRC or Driver’s License)

If Old passport additional:
