VACCINATION PROGRAM

WHEN: September 5, 2022 (MONDAY)
8:00 AM to 3:00 PM

WHERE: BARANGAY RISSING MULTIPURPOSE HALL

WHO:

•5-11 years old - first/second dose

•12-17 years old - first/second dose & booster dose (with 5 months interval from second dose)

•18 years old above
- first/second dose
- FIRST BOOSTER dose (all category group are eligible with 3 months interval from 2nd dose)

***SECOND BOOSTER DOSE (with 4 months interval from 1st booster dose) for the following category/age group:
- A1 (health workers)
- A2 (senior citizen)
- A3 (ONLY for 18-49 years old with comorbidity)
- 50 years old above (OPEN regardless of category)

Kindly bring the following:
- vaccination card (for 2nd dose/booster dose)
- birth certificate/valid id (for first dose)
-own ballpen
