🌊 Confirmed Flight Itinerary
🌊 DOT accredited Hotel
🌊 Negative RT PCR test or Saliva within 72hrs before departure.

🌊 Create S Pass account at https://s-pass.ph/ and apply for S Pass (after you apply, application should be on Pending status then Screen Capture)
🌊 Government Issued ID or any valid ID
🌊 Fill out Health Declaration form at touristboracay.com (Screen Cap the Confirmation page.)

Once all documents (Hotel and Flight Itinerary, S Pass with Pending Status, Negative RT-PCR or Saliva results, valid ID and Screen Capture of Health Declaration Form) are complete, you need to send it to [email protected]. If there are multiple travelers, you can send it in one Email.

Note: Email instruction is also in the final page once you've done filling out the Health Declaration form.

💫Check also your Spass, application status should be in Approved status.
