Most businesses in Nigeria don't get cheated by HMOs because they're careless. They get cheated because nobody gave them the right questions to ask. After helping hundreds of businesses and families predominantly in Lagos, Benin City, Asaba and Abuja find cover that actually works โ here is the exact checklist I give every client before they sign.
A business owner came to me 6 months after enrolling his 14 staff on a health plan.
His HR manager had fallen ill. Went to the hospital on their network list. Was turned away at the gate.
The hospital had been delisted 3 months earlier. The HMO never told him. Nobody did.
He paid the bills out of pocket. Then he renewed โ this time with ClearlineHMO.
That story is not rare. It is Tuesday for most Nigerian businesses.
Here's how to never be that person:
The brochure is marketing. The active hospital network list is operations. Ask for the updated version โ in writing. Then confirm your preferred hospital is still on it by calling the hospital directly. Takes 5 minutes. Saves you everything.
Every HMO plan in Nigeria has exclusions. Pre-existing conditions. Dental. Optical. Maternity. Surgeries above a certain cost. Know what's excluded before you're standing in a hospital needing it. The time to ask is not when your staff is already on a drip.
Who do you call in an emergency? Is pre-authorization needed before every visit? What happens on weekends and public holidays? If your HMO representative cannot answer this fluently and immediately, that hesitation is your answer.
A โฆ25,000/year plan sounds affordable. But if the annual benefit limit is โฆ150,000, a single admission for typhoid with complications will exhaust it. Always ask: what is the maximum payout per person, per year? Match it to your actual health risk.
Some HMOs offer low entry prices and significantly increase premiums at renewal โ especially after your team has made claims. Ask: "What is your renewal pricing policy?" If they will not put it in writing, walk away. That reluctance is a policy.

Save this checklist. Run every HMO plan you're evaluating through these 5 questions before you sign anything.
If you want me to personally walk you through how answers each one โ drop ๐๐๐๐๐๐๐๐ฆ๐ง in the comments or send me a DM.
Your staff deserve health cover that actually shows up when it matters. So does your business.

The meeting was virtual.
Four top decision makers on the other side of the screen.
A CFO. An HR Director. A MD. A legal rep.
People who run rooms.
People who have heard every pitch.
30 minutes.
That was all I had.
Six months before today I would never have dreamt been in that kind of room. Because I was not targeting room that size.
Last month I read something in ๐ง๐ต๐ฒ ๐๐ผ๐ฟ๐ฑ๐ ๐ผ๐ณ ๐ฆ๐๐ฟ๐ฎ๐๐ฒ๐ด๐ ๐ฏ๐ ๐ช๐ฎ๐น๐๐ฒ๐ฟ ๐๐ถ๐ฒ๐ฐ๐ต๐ฒ๐น that stopped me cold.
The biggest companies don't grow by working harder at the same level. They grow by ๐ฟ๐ฒ๐ฝ๐ผ๐๐ถ๐๐ถ๐ผ๐ป๐ถ๐ป๐ด โ choosing to operate in a bigger market tier.
So I asked myself one question: "What if I stopped going after small companies โ and only targeted businesses with 100+ staff?"
Same hours. Same skill. Different ceiling.

I painfully stopped calling companies with fewer than 100 staff.
Not because smaller companies don't matter.
But because I had to create a new floor for myself.
Most people never get to big deals because they are always available for small ones.
When you make yourself unavailable for small โ you start showing up differently for large.
Your conversations change. Your language changes. Your confidence changes.

I stopped sending generic emails and cold messages.
For every company I targeted, I spent between 10 to 30 minutes learning:
โ What they do
โ How many staff they have
โ Where their staff are located (especially field teams)
โ What their biggest operational challenges are
Then I wrote an email that was about ๐๐ต๐ฒ๐ถ๐ฟ ๐ฝ๐ฟ๐ผ๐ฏ๐น๐ฒ๐บ โ not my product.
The email that got me into yesterday's meeting didn't mention my company until the third paragraph.
The first two paragraphs were about them.
That is why it got opened. That is why it got a reply.
Once I got the meeting, I treated the preparation like the deal was already signed and I just had to not ruin it.
I knew their industry.
I knew the specific challenges their HR team faces.
I knew the exact questions a CFO asks before approving a spend this size.
I had answers ready before the questions were asked.
When you walk into a room โ virtual or physical โ knowing more about the prospect's world than they expect you to know...
The dynamic shifts.

Week 1 - 2 โ Decided to change strategy. Set the new minimum. Identified 36 target companies.
Week 3 - 4 โ Researched each one. Wrote personalized outreach. Sent 31 emails.
Week 5 - 7 โ Got 8 replies. Booked 6 meetings. Three cancelled.
Week 8 - 10 โ Two meetings happened. One became a โฆ14 million close and the other โฆ801K
Week 10 โ The multinational meeting was booked.
Week 11 โ Thursday. 27 million pitch. Decision pending but hopeful
77 days from a decision to โฆ15 million closed โ and โฆ27 million in the pipeline.
This time last year I didn't believe I could do โฆ15 million in 11 weeks. I definitely didn't believe I'd be in a โฆ27 million conversation.
Not because I got lucky.
Because I stopped playing a game that was too small for where I wanted to go.
But here is what i have learned:

The moment you decide to operate at a different level โ everything rearranges itself around that decision.
If you are reading this and you are stuck at a level that feels too small for you โ
The question is not
What is one decision you could make today that would change the floor of what you are willing to accept?
Drop it in the comments.
I want to read every single one.

Your company is paying for an HMO.
Your staff are still paying out of pocket.
And nobody is talking about it.
Let me tell you what actually happened to a logistics company in Lagos last year.
Their operations manager โ let's call him Chidi โ had a severe migraine attack on a Wednesday evening. He had an HMO card. 'Premium plan.' He went to the nearest accredited hospital.
They told him the authorization system was down.
He called the HMO helpline.
Nobody picked up.
He sat in that waiting room for 2 hours. In pain. Alone. With a card in his pocket that was supposed to protect him.
He eventually paid โฆ35,000 out of pocket and went home.
The next morning he sent his manager a receipt.
His manager sent it to HR.
HR escalated it to Finance.
Finance chased the HMO for 3 weeks.
The reimbursement never came.
โ
This story has 9 million versions across Nigeria every single year.
And the people living it are not complaining loudly โ they are quietly updating their CVs.
โ
75% of Nigeria's medical costs are paid out of pocket โ even with insurance (World Bank)
47 HMOs were sanctioned in 2024 for denying or delaying care (NHIA)
2M+ quietly absorbed per quarter in unplanned medical costs by the average mid-size business
โ
Here is the truth nobody in this industry wants to say out loud:
Most HMOs in Nigeria are not failing because of bad intentions. They are failing because hospitals don't trust them to pay โ and that distrust flows directly to your staff at the point of care.
When a hospital has been owed money by an HMO for 6 months, they develop a policy โ unofficial, never written down, but completely real: HMO patients wait longer. Get fewer drugs. See less senior doctors.
Your card didn't fail. The relationship behind your card failed.
โ
So what should you actually look for in a health plan? Here is your checklist โ share this with your HR team:
Does the HMO have a pharmaceutical partner?
If drugs aren't covered at the pharmacy, the consultation was pointless. Find out before you sign.
Do the hospitals in your HMO's network actually trust them?
Call two hospitals in the network. Ask if the HMO pays on time. You will learn more in 5 minutes than in 5 sales presentations.
03 โ Does telemedicine actually work โ or just exist on paper?
For field teams, remote workers, or anyone outside Lagos โ telemedicine is not a bonus. It is the whole system. Test it before you commit.
How old is the HMO?
A 3-year-old HMO has not yet proven it pays through an economic crisis. A 35-year-old one has survived multiple. That history is not nostalgia โ it is risk assessment.
When something goes wrong at 10pm on a Friday โ who exactly picks up?
Ask for the direct number of the person assigned to your account. If they hesitate, that is your answer.
โ
And for business owners who still think health insurance is 'just an HR expense':
Every time a talented employee leaves your company and says it was 'for better opportunities' โ run a quiet audit. How many of them mentioned healthcare in their exit interview? How many didn't mention it, but felt it every single month?
Inadequate healthcare is one of the top 3 silent drivers of employee attrition in Nigerian organisations โ and because it happens slowly, most leadership teams never connect the dots until the talent has already left.
Your health plan is not a perk.
It is a retention tool. A productivity infrastructure. A trust signal.
And in 2025, with the NHIA Act making it legally mandatory for every employer with 5+ staff โ it is also a compliance obligation you cannot ignore.

I am Azuka Orumgbe at ClearlineHMO โ 35 years of stable operations, backed by GreenLife Pharmaceuticals, with diagnostics through Clinix Healthcare across Africa.
We do not promise perfection. We promise a system that works the way it should โ at the hospital, at the pharmacy, at 10pm on a Friday, in a rural field location, wherever your people are.
If you want to benchmark your current plan or build a proper system from scratch โ DM me or drop a comment below. I will send you a no-obligation breakdown of what your team's coverage should actually look like.
P.S. โ If this resonated, share it with one HR manager or business owner in your network. One conversation could be the difference between a Chidi who stays and a Chidi who quietly starts looking.
