When you evaluate the case and go through the preoperative radiograph, you would find multiple reasons for failure and often neglect a crucial one!!

An unfilled isthmus in endodontics is a significant, often overlooked, cause of root canal treatment failure. It is a narrow, ribbon-shaped connection between two main root canals or roots that contains pulp tissue.

Because of its small size and complex anatomy, it frequently fails to be cleaned, shaped, and obturated, leaving a reservoir for bacteria and organic tissue that can lead to persistent infection.

Now this case was mainly tricky due to the fact that the case wasn't a typical c shaped anatomy, rather a root trunk/ pre furcation isthmus that was significantly clogged with debris and necrotic tissue.

Locating calcified root canals via freehand coronal access often leads to iatrogenic procedural errors such as deviations, perforations, and weakening of the remaining tooth structure.

These calcific deposits can cause a considerable challenge to the dental practitioner in locating the canals, reaching the working length and also cleaning and shaping the canals.

Traditional methods, particularly the use of rigid hand instruments (K files and C+ files) and 2D radiographic imaging, do not often work well when managing the complicated and narrow anatomy found in calcified root canals. These instruments cannot provide great flexibility and tactile sensitivity and expose operator error during these difficult situations.

We tend to use modern metallurgies and concepts managing these extremely complex calcified canals. Use of reciprocation ( symmetrical and asymmetrical oscillations along with continuous chelation concept- HEDP) works wonderfully well!!

Most important thing before attempting to negotiate such calcified canals is to have a thorough knowledge on the normal anatomical form and morphology of the root canal system of the particular tooth. Practicing under dental loupes or microscope can add to the clear vision and aid in locating and negotiating the canal.

Found this approach for locating mb2 really predictable.

By establishing the predicted location at the intersection of a line parallel to DB-P through MB and the perpendicular from DB to MB-P, the combined method overcomed the primary limitation of each individual method brought by the tendency of both methods to define a range rather than a specific single point.
