Today, bail applications of three accused named

Ajit S/o Sahib lal Jain, Sanat S/o Manak jain and
Pratham S/o Poonamchand

was posted for hearing in the court of DJ, Udaipur.

Since DJ was on leave, the bail applications were transferred to and heard by the court of ADJ NO. 5, Udaipur.

Four lawyers appeared on our behalf.

As earlier, a bunch of lawyers appeared on behalf of all three accused.

*Important Anesthesia Alert: GLP-1 Receptor Agonists & Aspiration Risk*

More patients are now using GLP-1 RA injections for weight reduction, such as Mounjaro®️ (Tirzepatide) and Wegovy®️ (Semaglutide). These are antidiabetic drugs that significantly delay gastric emptying.

*Clinical Implication:*
This delayed emptying poses a high risk of pulmonary aspiration under anesthesia, _even with standard NBM status_.

*Case Observation:*
Patient received her last dose 3 weeks ago and was NBM since 4:00 PM the previous day. On pre-induction gastric ultrasound, the antrum was still full with semi-digested solid food particles.

*Key Message for Anesthetists:*

1. *Treat as Full Stomach:* Any patient on GLP-1 RAs should be considered _not NBM_ for the purpose of aspiration risk, regardless of fasting duration or time since last dose.
2. *Use Gastric Ultrasound:* If available, scan the gastric antrum preoperatively to assess content and volume.
3. *Airway Management Strategy:*
- *Consider postponing* elective surgery if risk outweighs benefit.
- *Empty stomach* with NG tube when feasible.
- *Use Rapid Sequence Induction* with cricoid pressure.
- *Avoid Supraglottic Airway Devices* — secure the airway with endotracheal intubation.

*Summary:* Standard fasting guidelines do not apply to these patients. Assume delayed gastric emptying and plan for full stomach precautions.
