Symptoms and Complications

Papilledema is swelling of your optic disc, the place where the nerve that goes to your brain and connects inside your eye. This happens due to pressure buildup in or around your brain.

Mostly it's due to some other serious problem which needs to be diagnosed and treated. Sometimes the pressure and swelling can't be traced to a specific problem. In such a case, there are other ways to ease the swelling.
If left untreated, papilledema can lead to vision loss. This increased pressure can cause severe encephalitic complications like abscess, tumours, meningitis or encephalitis, which may lead to a patient’s death.

When there's no apparent reason for high pressure inside your skull, the condition is called idiopathic intracranial hypertension (IIH).

In the early stages, papilledema may be asymptomatic or present with a headache. It can progress to enlargement of the blind spot, blurring of vision, visual obscurations. Ultimately, total loss of vision can occur.

In literature, there have been several papilledema case studies carried out from a medical perspective, where papilledema was found along with the presence of some life-threatening diseases, thus helping doctors in their diagnosis. It is recommended that every patient of papilledema should be treated very carefully.

Diagnosis

Doctors use a tool called an ophthalmoscope to look inside the back of the eyes and diagnose papilledema. An imaging test, such as an MRI, can provide more details and possibly show what's causing the pressure in the brain. Later on, MRIs can measure how well treatment is working.

When papilledema is found on fundoscopy, further evaluation is warranted. Recent research has shown that point-of-care ultrasound can be used to measure optic nerve sheath diameter for detection of increased intracranial pressure and shows good diagnostic test accuracy compared to CT.

Treatment

Treatment of papilledema can be very different from patient to patient depending on each situation. For instance, a patient might need antibiotics for a brain infection, surgery to drain an abscess or remove a tumour, or medicine to dissolve a blood clot.

The symptoms will likely guide the treatment. With slight papilledema and no symptoms, the doctor might simply keep checking and do regular testing to spot any vision problems as soon as possible. Even losing weight might help with the condition as obesity might be the cause.

Angioplasty and percutaneous transluminal angioplasty is a minimally invasive endovascular procedure to widen narrowed or obstructed arteries or veins, typically to treat arterial atherosclerosis.

A stent is a tiny tube that the doctor can insert into a blocked passageway to keep it open. The stent restores the flow of blood. Stents are usually needed when plaque blocks a blood vessel. Plaque is made of cholesterol and other substances that attach to the walls of a vessel.

A coronary artery stent is a small, metal mesh tube that expands inside a coronary artery. A stent is often placed during or immediately after angioplasty. It helps prevent the artery from closing up again. A drug-eluting stent has medicine embedded in it that helps prevent the artery from closing in the long term.

Coronary angioplasty is a therapeutic procedure to treat the stenotic narrowed coronary arteries of the heart found in coronary heart disease. These stenotic segments are due to the buildup of cholesterol-laden plaques that form due to atherosclerosis. A stent is inserted along with a balloon catheter. It expands when the balloon is inflated. The stent is left there to help keep the artery open.
