Breathlessness (Dyspnoea) is a common symptom to which many have had experienced after hard exercise or after climbing stairs . But breathlessness arise when you are at rest or sleeping , it is very troublesome. Such episodes of breathlessness should be investigated and requires urgent medical care under a physician . They give an indication of heart , lungs or a metabolic disease. Onset of breathlessness may be sudden ( acute dyspnoea) or sometimes it is provoked by even with mild exertion (Cronic exertional dyspnoea ) causes of acute dyspnoea are acute pulmonary oedema, acute severe asthama. Acute exacerbation of COPD , pneumothorax, pneumonia, pulmonary embolus, Acute respiratory distress syndrome (ARDS), anaphylaxis, diabetic ketoacidosis. etc Cronic exertional dyspnoea is due to cronic heart failure, Myocardial ischemia, COPD, cronic asthama, bronchial carcinoma , pleural effusion, fibrosing alveolitis, interstitial lung disease, severe anaemia etc.

Increasing attention is being focused on short - chain fatty acids ( SCFAs) that are produced in the colon and absorbed from it. SCFAs are two to five carbon weak acids that have an average normal concentration of about 80 mmol/ L in the lumen. About 60 percent of this total is acetate , 25 percent propionate, and 15 percent buterate. They are formed by the action of colonic bacteria on complex carbohydrates, resistant starches, and other components of dieatry fibre, that is material that escapes digestion in upper gastrointestinal tract and enters the colon . Absorbed SCFAs are metabolized and make a significant contribution to total caloric intake . In addition , they exert a trophic effect on colonic epithelial cells, combat inflammation , and are absorbed by exchange for H+ , helping to maintain acid base equilibrium. SCFAs are absorbed by specific transporters present in colonic epithelial cells .
