We’re recruiting: PCN Cancer Care Coordinator

Do you want to make a real difference for people living with cancer in Nottingham?
We’re looking for a Cancer Care Coordinator to join Self Help UK,
• Working in partnership with Clifton & The Meadows Primary Care Network.
• Support people following a cancer diagnosis
• Deliver Cancer Care Reviews & Holistic Needs Assessments
• Work across GP practices, communities and the voluntary sector

Nottingham | Hybrid | 30 hrs | Fixed term until April 2027, with possibility of extension
