1️⃣ Go to https://www.express-scripts.ca/downloads-and-resources/dentists-downloads
2️⃣ Download the Dental Provider Modification Form.
3️⃣ Submit it to Express Scripts — either by fax or upload it using the Provider Contact Form. (https://www.express-scripts.ca/healthcare-providers/become-a-provider)
