Families will meet one-on-one with an AccessOAP Care Coordinator for personalized support with their OAP journey.

🔹 Date: Wednesday May 27
🔹 Time: 45 minute appointments between 9am – 4pm

For registration support please bring the required documents:
🔹 Proof of age
🔹 Proof of residency in Ontario
🔹 Proof of diagnosis

🔹 Prefer to meet with someone in-person
🔹 Need assistance submitting OAP expenses
🔹 Would like to speak with someone about the OAP and the services and supports available after registration
