Déformation faciale

Un jeune cheval nous a été présenté avec une déformation notable du côté gauche du visage. Afin de déterminer la cause sous-jacente de cette tuméfaction, un scanner (CT) de la tête a été réalisé.

L'imagerie a révélé un kyste sinusal, une lésion bénigne mais occupant de l'espace, se développant dans l'un des sinus du crâne. Ces kystes sont remplis de liquide et entourés d'une fine membrane. Les kystes sinusaux surviennent plus fréquemment chez les jeunes chevaux et peuvent se développer aussi bien dans les sinus maxillaires que frontaux.

À mesure que le kyste grossit, il exerce une pression sur l'os environnant, ce qui peut entraîner une déformation faciale visible, comme observé chez ce cheval. Selon la localisation et la taille, un kyste sinusal peut également provoquer d'autres signes cliniques, tels qu'un écoulement nasal unilatéral, une respiration anormale, des problèmes dentaires ou encore une pression sur le canal lacrymal entraînant un larmoiement de l'œil.

Dans ce cas, le cheval présentait effectivement des difficultés respiratoires et son crâne était déjà visiblement déformé à son arrivée. Pour résoudre ce problème, le kyste a été retiré chirurgicalement via un petit volet créé dans le sinus, à travers le crâne. Cette approche a permis de retirer la masse et nous a donné un bon accès pour nettoyer soigneusement le reste de la cavité sinusale.

Facial deformation

A young horse was presented to our clinic with a noticeable deformation on the left side of the face. To determine the underlying cause of this swelling, a CT scan of the head was performed.

The imaging revealed a sinus cyst, a benign but space occupying lesion that develops within one of the skull's sinuses. These cysts are fluid filled and surrounded by a thin membrane. Sinus cysts occur more frequently in young horses and can develop in both the maxillary and frontal sinuses.

As the cyst grows, it places pressure on the surrounding bone, which can lead to a visible facial deformation, as seen in this horse. Depending on the location and size, a sinus cyst can also cause other clinical signs, such as one sided nasal discharge, abnormal breathing, dental problems or even pressure on the tear duct causing the eye to water.

In this case, the horse indeed showed difficulty breathing, and her skull was already visibly deformed at the time of presentation. To resolve this, the cyst was surgically removed through a small flap created in the sinus via the skull. This approach allowed the mass to be removed and gave us good access to thoroughly clean out the remaining sinus cavity.

Gezichtsvervorming

Een jong paard werd bij ons aangeboden met een opvallende vervorming aan de linkerzijde van het gezicht. Om de onderliggende oorzaak van deze zwelling te achterhalen, werd een CT-scan van het hoofd uitgevoerd.

De beeldvorming toonde een sinuscyste, een goedaardige maar ruimte-innemende afwijking die ontstaat in een van de sinussen van de schedel. Deze cystes zijn gevuld met vocht en omgeven door een dun membraan. Sinuscystes komen vaker voor bij jonge paarden en kunnen zowel in de kaakholtes als in de voorhoofdsholtes ontstaan.

Naarmate de cyste groeit, oefent ze druk uit op het omliggende bot, wat kan leiden tot een zichtbare vervorming van het gezicht, zoals bij dit paard het geval was. Afhankelijk van de locatie en grootte kan een sinuscyste ook andere klachten veroorzaken, zoals eenzijdige neusuitvloei, een abnormale ademhaling, tandproblemen of zelfs druk op de traanbuis waardoor het oog gaat tranen.

In dit geval vertoonde het paard inderdaad ademhalingsmoeilijkheden en was haar schedel bij aankomst al zichtbaar vervormd. Om dit op te lossen werd de cyste chirurgisch verwijderd via een klein luikje dat in de sinus, doorheen de schedel, werd gemaakt. Deze aanpak liet toe om de massa te verwijderen en gaf ons goede toegang om de resterende sinusholte grondig te reinigen.
