OCT INTERPRETATION

Interpreting an OCT (Optical Coherence Tomography) scan involves analyzing cross-sectional images of the retina, especially the macula and optic nerve head. Here’s a basic guide to OCT interpretation, focusing on the retina and macula:

1. Normal OCT Appearance
★Foveal depression: A healthy dip at the center of the macula.
★Clear retinal layers: The retina shows multiple well-defined, distinct layers.
★Normal thickness: Central macular thickness typically ranges from 250–300 microns (may vary slightly by device).

2. Common Pathologies
A. Age-Related Macular Degeneration (AMD)
★Dry AMD: Drusen (yellow deposits) appear as small, dome-shaped elevations beneath the retinal pigment epithelium (RPE).
★Wet AMD: Presence of subretinal/intraretinal fluid or hemorrhage; pigment epithelial detachment (PED); hyperreflective lesions.

B. Diabetic Macular Edema (DME)
★Cystoid spaces within the retina.
★Diffuse retinal thickening.
★Hard exudates may appear as hyperreflective spots.

C. Central Serous Retinopathy (CSR)
★Serous detachment of the neurosensory retina with a clear fluid pocket beneath it.
★RPE changes or PED may be present.

D. Epiretinal Membrane (ERM)
★Hyperreflective membrane on the inner surface of the retina.
★Retinal wrinkling or distortion.
★Loss of foveal contour.

E. Macular Hole
★Full-thickness defect in the central macula.
★Surrounding cystoid changes or detachment of the posterior vitreous.

F. Glaucoma (Optic Nerve OCT)
★Thinning of the RNFL (Retinal Nerve Fiber Layer).
★RNFL maps show color-coded areas: green = normal, yellow = borderline, red = abnormal.
★Cup-to-disc ratio may be increased.

3. Key Measurements
A. Central macular thickness (CMT): Assesses macular edema.
B. Retinal Nerve Fiber Layer (RNFL) thickness:
C. Used in glaucoma evaluation.
D. Ganglion Cell Layer (GCL): Also important in optic neuropathies.

Tips for OCT Interpretation
1.Compare with normative data (color-coded reference maps).
2.Look for symmetry between eyes.
3.Use serial OCTs to monitor progression or treatment response.
