Did you know that cardiovascular disease is often preventable when risk factors are identified early?

A Heart Health Check is available for:
✅ Aboriginal and Torres Strait Islander peoples aged 30 years and over
✅ Adults aged 45 years and over

During your appointment, your GP will:
🩺 Review your medical and family history
🩺 Assess risk factors such as smoking, alcohol intake, diabetes and blood glucose levels
🩺 Check your blood pressure and cholesterol status
🩺 Calculate your absolute cardiovascular disease risk
🩺 Develop a personalised management plan if needed
🩺 Provide advice on healthy lifestyle changes to help protect your heart

Due to a scheduled power outage, we will be opening later than usual on Wednesday 27th, with doors opening and phones turning on at 10:30am.

Did you know you may be eligible for a GP Care Plan?

If you have a chronic health condition that has lasted (or is expected to last) longer than 6 months, you may qualify for a care plan with your GP. This can include conditions such as arthritis, diabetes, asthma, COPD, chronic pain, high blood pressure, high cholesterol, plantar fasciitis, behavioural/developmental concerns, and more.

A GP Care Plan lasts for 12 months and gives eligible patients access to up to 5 Medicare-subsidised allied health visits per calendar year.

✅ Care plans are fully bulk billed for patients with a valid Medicare card
✅ Most allied health services are also bulk billed
