For many of us, a simple text or calendar alert is all it takes to remember an appointment.

But what if you don’t have a phone? What if there’s no stable address where reminders can reach you?

We understand that life can be unpredictable, that’s why we're building a system, so you are not forgotten.

📌 𝗢𝘂𝘁𝗿𝗲𝗮𝗰𝗵 𝗮𝗻𝗱 𝗙𝗮𝗰𝗲-𝘁𝗼-𝗙𝗮𝗰𝗲 𝗥𝗲𝗺𝗶𝗻𝗱𝗲𝗿𝘀
– Our team works closely with community caseworkers, outreach services, and hospitals to check in and remind clients of their appointments. In some cases, community health nurses and caseworkers can help bring clients to their appointments, ensuring that transportation or logistical barriers never stand in the way of receiving care.

📌 𝗣𝗮𝗿𝘁𝗻𝗲𝗿𝘀𝗵𝗶𝗽𝘀
– We collaborate with homeless relief services who know the community and can help make sure messages get through.

📌 𝗙𝗹𝗲𝘅𝗶𝗯𝗶𝗹𝗶𝘁𝘆 𝗮𝗻𝗱 𝗨𝗻𝗱𝗲𝗿𝘀𝘁𝗮𝗻𝗱𝗶𝗻𝗴
– If someone misses an appointment, we follow up.

Catherine ( in the picture below), from VincentCare, has worked tirelessly, not only referring countless homeless clients to Mobile GP, but also ensuring they don't miss an appointment, even when they’ve moved into housing elsewhere. She’s the one who makes sure they get there. From navigating transportation barriers to reminding them when it’s time to see a doctor, Catherine is the steady hand that guides them, no matter where life takes them.

It was so cool to catch up with Rama today.

Saying we’re inspired is an understatement.

In her 80s, she had one goal: to visit her family in London and explore Europe. But first, she needed to make sure her health wouldn’t hold her back.

Through the Mobile GP program and Mark & Sylvie's Home Care , she got exactly what she needed, a telehealth GP, an onsite nurse for her B12 injections, a face-to-face geriatrician, and a plan tailored to her needs.

The result? Life-changing.

She didn’t just visit her family, she spent two months traveling across 12 European countries. And here’s the incredible part:

She started by walking upto 5km a day. By the end of her journey it was 12-13km daily.

Amazing! 🤩

Rama now understands how to manage her health, giving her the freedom to live on her terms.

Lesson: Yes, healthcare is about treating illness, but it’s also about unlocking possibility. ✨

What does holistic care really mean?

Let’s get personal.

Imagine you’ve been carrying a heavy backpack for years. It’s crammed with life’s weight, mental health struggles, physical pain, and the emotional toll of uncertainty. Now, imagine someone offers to help, but they only pull out one item. Your back still hurts.

This is why holistic care matters. It’s not about putting a Band-Aid on one part of your life. It’s about unpacking the whole bag: your body, your mind, and your heart.

For our Mobile GP patients, this approach can be life-changing. It’s about listening to more than just symptoms, it’s hearing the story behind them. It’s connecting dots between mental health and physical health, between isolation and stability.

If you’re feeling stuck, here’s what you can do today:

1️⃣ Ask for help—without shame. Speak to someone who will see all of you, not just your problem.
2️⃣ Move your body—even a short walk can release tension. Physical health is a gateway to emotional health.
3️⃣ Be kind to yourself—progress takes time, but you’re worth the effort.

Holistic care is a necessity. And together, we can lighten the load for those carrying far too much.
