🍹 In this Ultrasound Happy Hour, we are presenting you with the case of a 66-year-old survivor of sudden cardiac death with extensive scarring from previous anterior and inferior myocardial infarctions due to multi-vessel coronary artery disease (CAD).

➡️ ap4CV with CD: Severe LV dilatation and presence of ICD leads in the right heart (standard secondary prevention for SCD survivors).
➡️ ap2CV: The inferior wall is thin and highly echogenic (bright), consistent with the patient's history.
➡️ Apical 4CV: Note the spontaneous contrast (swirling "smoke") due to slow flow (a major red flag for high thrombus risk).

CAVEAT: An apical aneurysm is identified as a distinct, thin-walled segment at the left ventricular apex exhibiting dyskinetic or akinetic behavior. It's important to recognize the subtle progression from apical dilatation to aneurysm formation!
