When there are not clear and enforced rules about where patients should get primary care services, it can lead to them seeking this care at their local hospital where it likely will be more expensive and lead to long waiting times.
Gatekeeping is the process of guiding patients first to their local primary care providers to optimize efficiency and ensure continuity. We just released a series of reports analyzing how gatekeeping works in developing countries and how financial mechanisms, among other health systems levers, are essential to ensuring that it works well.

We capped off 2024 with a celebration of our SP4PHC project completion in Uganda! For the last five years, our team has worked intensively with the Government of Uganda to achieve major progress towards universal health care in Uganda:
✅Filled evidence gaps around PHC financing, complex fund flows, and the impact of decentralization and national public financial management on the health system
✅ Laid out opportunities for donor engagement (which plays a large role in Uganda), including addressing the challenge of patient congestion at public PHCs in Kampala
✅ Helped the government design, implement, and evaluate its mainstreaming of a World Bank-funded RBF project, called the Uganda Reproductive, Maternal and Child Health Improvement Project

Earlier this month, ThinkWell visited Pakistan to kick off our study on the cost of identifying and vaccinating zero-dose children, a collaboration with Precision Health Consultants Global.
