You will be traveling within a two-hour travel radius from the main location within which you will be visiting the patient's in-home. 80%+ of the job is on the road visiting patients. Are you comfortable with this and willing to put in the road time?
preferred
2+ years experience with any of the following:
Dialysis Care
Home Health Care
Hospice
Case Management (CM)
Are you bilingual in English/Spanish?

required Valid NP certification/license in Pennsylvania (PA)
Valid Drug Enforcement Administration (DEA) certification/license
NP (Independent Practice) (2+ years)
2+ years experience with any of the following:
Home Health Care
Hospice
Managed Care (2+ years)
Do you have experience with computer-based data management programs?
Are you fully vaccinated?
Do you have experience conducting annual wellness visits or similar comprehensive visits and appropriately charting HCCs; general knowledge of risk adjustment.
