How It Works:
1. Have an introductory meeting by phone or in person to meet with family and gain a better understanding about their loved one.
2. Schedule a time where we visit their senior living community to meet resident, staff, RN, and CNA. While there, family signs paperwork allowing me to visit the resident and communicate with care team.
3. Schedule appointment where I go to the community and check on their family member. While visiting, I will document and make a report of the following:
* General overview of resident and their living conditions: Is their room clean? Have sheets been changed? Has the resident showered? Is the resident eating all meals? Do they need, snacks, supplies, etc.?
* Visit with RN: How is the health of the resident? Have they been taking all medication? Are there any concerns with resident?
* Visit with CNA or caregiver: Does the resident need a lot of help? Is the resident allowing you to help them? How does the resident's health seem to you?
4. From the information above, I will generate a one page report and email the report to the family. I will then call and schedule a time to review findings with family.

Take a picture of the resident.
Share a meal with resident to help build the relationship.
FaceTime resident with family.
