Site 2: Brgy. Cabanbanan Covered Court
*Adult First Dose:
Janssen Vaccine (1 Dose Only)
*Adolescent (12 to 17 Years Old)
First Dose: Moderna Vaccine
*Booster Dose: Moderna Vaccine

Patients Eligible for Booster
*18 years old and above who have been vaccinated
at least 3 months ago with astrazeneca,pfizer,moderna,sinopharm,
sinovac and sputnik(gamaleya)
*18 years old and above who have been vaccinated with janssen at least 2 months ago.

Requirements:
1. ID
2. Vaccination Card

Adult First Dose:
Requirement: ID

Adolescent first dose (12 to 17 years old) requirements:
1. Photocopy of ID of the Child and Photocopy of ID of the Guardian(If the child has no ID, Brgy. Certificate will be accepted, attached with a photocopy of the child's birth certificate)

2. If no parent is present, and the child is only with a guardian bring authorization letter signed by one of the parents, photocopy of ID of the parents and/or Brgy. Certificate of Guardianship

3. If with Comorbidity - Bring Medical Clearance
