It is important to update your beneficiary nomination form whenever your personal circumstances change. This includes events such as marriage, divorce, the birth of a child, or the death of a nominated beneficiary. In such cases, a new form must be completed at least once every year to ensure that your details remain accurate and up to date.

The form can be obtained from your employer or at any Old Mutual branches. Once completed, a copy must be submitted to your employer for safekeeping. Alternatively, you may update your beneficiary nomination electronically by logging onto the Old Mutual Member Web Portal at www.oldmutual.co.za. Completed forms may also be emailed to [email protected].
