Psoriasis isn’t just a skin condition — it’s a chronic, immune-mediated, systemic inflammatory disorder, that you must master for the exam. We will know about it's various types, there will be quiz at the end of the post.

1️⃣ 𝐏𝐥𝐚𝐪𝐮𝐞 𝐏𝐬𝐨𝐫𝐢𝐚𝐬𝐢𝐬 (Vulgaris) – Most common ✅
🔹 Well-demarcated erythematous plaques + silvery white scales
🔹 Auspitz sign = pinpoint bleeding on scratching
🔹 Sites: elbows, knees, scalp, lower back, umbilicus, ears (face ⬇️, but more in kids)

2️⃣ 𝐆𝐮𝐭𝐭𝐚𝐭𝐞 𝐏𝐬𝐨𝐫𝐢𝐚𝐬𝐢𝐬 – “Drop-like” lesions 💧
🔹 Small, scaly plaques
🔹 Often after strep throat (think 👩‍🎓 sore throat → skin rash)

3️⃣ 𝐈𝐧𝐯𝐞𝐫𝐬𝐞 (𝐅𝐥𝐞𝐱𝐮𝐫𝐚𝐥) 𝐏𝐬𝐨𝐫𝐢𝐚𝐬𝐢𝐬 – Hidden folds 🔄
🔹 Axilla, groin, inframammary, intergluteal cleft
🔹 No scale (moist areas)

4️⃣ 𝐏𝐚𝐥𝐦𝐨𝐩𝐥𝐚𝐧𝐭𝐚𝐫 𝐏𝐬𝐨𝐫𝐢𝐚𝐬𝐢𝐬 – Thick skin, tough to treat! ✋🦶
🔹 Dry, scaly patches or pustules on palms/soles

5️⃣ 𝐏𝐬𝐨𝐫𝐢𝐚𝐭𝐢𝐜 𝐍𝐚𝐢𝐥 𝐃𝐢𝐬𝐞𝐚𝐬𝐞 – Exam favorite! 💅
🔹 Pitting, oil spots, onycholysis, subungual hyperkeratosis
🔹 Red flag 🚩 – ↑ risk of psoriatic arthritis

6️⃣ 𝐏𝐮𝐬𝐭𝐮𝐥𝐚𝐫 𝐏𝐬𝐨𝐫𝐢𝐚𝐬𝐢𝐬 – Severe variant ⚠️
🔹 Sterile pustules (localized or generalized)
🔹 Generalized (Von Zumbusch) = medical emergency 🚑

7️⃣ 𝐄𝐫𝐲𝐭𝐡𝐫𝐨𝐝𝐞𝐫𝐦𝐢𝐜 𝐏𝐬𝐨𝐫𝐢𝐚𝐬𝐢𝐬 – Life-threatening ❗
🔹 Diffuse redness & scaling over most of body
🔹 Risk of high-output cardiac failure, sepsis

🔍 All About the NAIL PLATE:

· Hard 💪
· Convex (Curved)
· Rectangular ◼️
· Translucent (That's why you see the pink nail bed underneath!)
· Thickness: 0.5-0.7 mm

📏 It's Divided into 3 Parts Lengthwise:

(1) Proximal Portion➡️ চামড়ার নিচের শক্ত অংশটুকু।

(2) Part on the Nail Bed➡️ The main, visible pink part.

(3) Distal Free Edge➡️ The white tip we clip! ✂️

Hey everyone! 👋 Let's dive deep into understanding what's really happening under your skin when you experience ! 🧐 It's much more than just a surface problem; it's a complex interplay of several factors within the pilosebaceous units.

Four principal events are the key players in acne's development:

• 1. 🛡️ 𝐅𝐨𝐥𝐥𝐢𝐜𝐮𝐥𝐚𝐫 𝐇𝐲𝐩𝐞𝐫𝐤𝐞𝐫𝐚𝐭𝐢𝐧𝐢𝐳𝐚𝐭𝐢𝐨𝐧: This means abnormal growth of skin cells lining the hair follicle, which leads to follicular plugging. Essentially, your pores get clogged! This is the earliest, subclinical lesion, the microcomedo, which can turn into blackheads or whiteheads.

• 2. 💧 𝐈𝐧𝐜𝐫𝐞𝐚𝐬𝐞𝐝 𝐒𝐞𝐛𝐮𝐦 𝐏𝐫𝐨𝐝𝐮𝐜𝐭𝐢𝐨𝐧: Behind that pesky plug, the sebaceous glands ramp up their oil production, leading to excess sebum. This creates a prime environment for bacteria!

• 3. 🦠 𝐏𝐫𝐨𝐩𝐢𝐨𝐧𝐢𝐛𝐚𝐜𝐭𝐞𝐫𝐢𝐮𝐦 𝐚𝐜𝐧𝐞𝐬 (𝐏. 𝐚𝐜𝐧𝐞𝐬) 𝐏𝐫𝐨𝐥𝐢𝐟𝐞𝐫𝐚𝐭𝐢𝐨𝐧: This bacterium is a normal skin resident, but with all that extra sebum and follicular plugging, it starts to multiply rapidly within the pilosebaceous unit. 𝘗. 𝘢𝘤𝘯𝘦𝘴 then breaks down sebum into free fatty acids, irritating the follicular wall and kicking off inflammation. It also ramps up inflammatory responses and can even cause the follicle to rupture! 💥

• 4. 🔥 𝐈𝐧𝐟𝐥𝐚𝐦𝐦𝐚𝐭𝐢𝐨𝐧: This is your body's immune response to all the irritation and bacterial activity. It's what causes the characteristic redness and swelling you see in acne lesions, attracting immune cells and worsening the situation.

𝐖𝐡𝐚𝐭 𝐞𝐥𝐬𝐞 𝐟𝐮𝐞𝐥𝐬 𝐭𝐡𝐢𝐬 𝐩𝐫𝐨𝐜𝐞𝐬𝐬?

• 🧬 𝐇𝐨𝐫𝐦𝐨𝐧𝐞𝐬 (𝐀𝐧𝐝𝐫𝐨𝐠𝐞𝐧𝐬): Especially during puberty, androgens (like testosterone and DHT) are major culprits! They stimulate both keratinocytes (skin cells) and sebaceous glands, leading to more sebum production and follicular proliferation.
Clinical pearl: Men with 𝐚𝐧𝐝𝐫𝐨𝐠𝐞𝐧 𝐢𝐧𝐬𝐞𝐧𝐬𝐢𝐭𝐢𝐯𝐢𝐭𝐲 don't produce sebum and thus don't get acne!

• 🍎 𝐃𝐢𝐞𝐭𝐚𝐫𝐲 𝐂𝐨𝐧𝐧𝐞𝐜𝐭𝐢𝐨𝐧𝐬: Your diet can play a role too! High glycemic index (GI) and glycemic load (GL) diets can lead to increased insulin and IGF-1 levels. This, in turn, can stimulate the proliferation of skin cells and sebaceous glands, and even activate androgens, all contributing to acne development.

Understanding these interconnected factors is a big step towards effectively managing and treating acne! ✨

ডার্মাটোলজির Lesions দিয়েই শুরু করা যাক! কারণ স্কিন ডিজিজের বর্ণনাই শুরু হয় Lesion দিয়ে।Lesion দুই ধরনের:

1️⃣ Primary Lesions (সরাসরি ডিজিজের কারণে তৈরি হয়)
এটাকে সহজে মনে রাখতে আমরা কয়েকটা ভাগে ফেলতে পারি

★Lesion এর সারফেস হবে মসৃণ,শুধুমাত্র রঙ এর পরিবর্তন হবে।
√1cm এর ছোট হলে > Macule
√1cm বা তার বড় হলে > Patch. যেমন,Vitiligo.

★Lesion এর সারফেস হবে উঁচু, সলিড, ভেতরে ফ্লুইড থাকবে না।
√1cm এর ছোট হলে > Papule. যেমন Acne
√1cm বা তার বড়, তবে ওপরটা ফ্ল্যাট > Plaque. যেমন, Leprosy.
√1cm বা তার বড়, ওপরে round > Nodule.
যেমন Xanthoma

★Lesion এর সারফেস হবে উঁচু, তবে ভেতরে ফ্লুইড
√1cm এর ছোট হলে > Vesicle. যেমন Chicken Pox.
√1cm বা তার বড় > Bullae. যেমন, Pemphigus vulguris
√Fluid filled lesion, but it's pus > Pustule.
যেমন Bacterial infections.

2️⃣ Secondary Lesions (প্রাইমারি Lesion এর পরবর্তী স্টেজ/Scratching/Trauma থেকে হয়)

★Scale (Dry or greasy, skin pilling, kerarin mass.যেমন Psoriasis)

★Crust (Skin lesion থেকে serum, pus, blood বের হয়ে Dry হয়, এবং একটা layer তৈরি করে, যেমন bacterial infections)

★সবশেষে আসি স্কিন destruction নিয়ে,

√Loss of epidermis, disease process > Erosion

√Loss of epidermis, mechanical process like scratching/trauma > Excoriation/Abrasion

√Loss od epidermis+dermis > Ulcer

√Linear cleft (ফাটল) in epidermis > Fissure, যেমন Eczema.

√Replacement of lost skin tissue with fibrous tissue > Scar, সাধারণত dermis/subcutaneous tissue damage হলে Scar formation হয়।
